Your Start My Care Form is Complete!

What Happens Next?

Thanks again for completing your start my care form — you’re almost set!

Here’s what happens next:
✔ We’ll contact you to schedule your first appointment
✔ We’ll verify your insurance (if you’re using it)
✔ You’ll receive your appointment link and reminders

A copy of your completed form is now available in your secure patient portal:

https://televerohealth.com/portal

If you haven’t already, you can create your portal account using the same phone number, date of birth, and email address you used when registering.

Why it matters: The portal gives you access to your completed forms, upcoming appointments, documents, secure messages, and more — all in one place.

Registering on behalf of someone else? Once the portal account is set up using the patient’s information, a legal parent or guardian can be added by calling our office.

🗓 Next Step: Scheduling Your Appointment

Our team will reach out within one business day to schedule your first appointment at a time that works for you.

If you’re not using insurance:

We’ll connect you with a provider based on your availability and preferences.

If you’re using insurance:

We’ll start by verifying your behavioral health benefits. This process can take up to two business days, depending on how quickly your insurance provider responds. Most patients get their information in about a day.

What we check: We’ll confirm if your plan covers behavioral health, whether a referral or prior-authorization is needed, and how much your plan is likely to cover for you.

Good to know: You won’t be charged for anything until your benefits are confirmed and clearly explained to you.

If it turns out your insurance doesn’t cover behavioral health services, you’ll have the option to cancel your appointment at no cost.

Once benefits are verified, a team member will contact you to review your coverage and confirm your first appointment date and time.

💼 A Note About Insurance Coverage

We’re here to help you get the most from your behavioral health benefits. While we verify your coverage and advocate on your behalf, please know that you are responsible for any portion your insurance doesn’t cover or denies.

🧑‍⚕️ Optional Support With Medication

We have licensed providers on staff who can support you with medication management — if that is currently part of your care plan or becomes part of your care plan. If you’re already working with another doctor, we’re happy to coordinate with them to ensure continuity of care.
